DPH + DXM
Diphenhydramine (DPH) + DXM: the short answerCombining Benadryl with DXM ('robo + DPH') stacks delirium on dissociation and raises the risk of seizures, dangerous overheating, and heart-rhythm problems. This is one of the worst over-the-counter combinations.
Why it’s dangerous
DXM is a dissociative with serotonin activity; diphenhydramine is strongly anticholinergic and sedating. Together they produce profound confusion and delirium, raise body temperature and heart rate, lower the seizure threshold, and add serotonergic pressure — a mix that lands people in emergency departments with agitation, hyperthermia, and cardiac rhythm disturbances.
What happens
- Intense, frightening delirium with realistic hallucinations
- Severe confusion and inability to communicate
- Racing heart, high blood pressure, and overheating
- Seizures and, rarely, life-threatening arrhythmias
Warning signs
- Extreme agitation or unresponsiveness
- Hot, dry, flushed skin with very fast pulse
- Muscle jerking, rigid limbs, or seizure
What to do
- Call emergency services for seizures, severe agitation, or very high temperature
- Keep the person cool and away from hazards; do not leave them alone
- Do not combine any two over-the-counter 'trips' — the risks multiply instead of adding
- Tell medical staff exactly what and how much was taken; it changes treatment
