DXM + SSRIs
DXM + SSRIs: the short answerRobotripping while on an SSRI can cause serotonin syndrome — cough medicine and antidepressants are a genuinely dangerous pair. Agitation, sweating, and stiff, jerking muscles after this mix is an emergency.
Why it’s dangerous
Dextromethorphan raises serotonin by blocking its reuptake and promoting its release; SSRIs block reuptake too. Combined, serotonin can climb to toxic levels, and the effect is stronger at recreational DXM doses. Some people also metabolize DXM slowly (or take fluoxetine/paroxetine, which block its breakdown), making the DXM itself hit much harder and last much longer.
What happens
- Serotonin syndrome: agitation, confusion, sweating, diarrhea
- High fever, rigid muscles, and jerking movements
- A DXM trip that is far more intense and prolonged than expected
- Seizures in severe cases
Warning signs
- Distress, tremor, and sweating early in the experience
- Temperature above ~38.5°C/101°F with stiff muscles
- Racing heart, very high blood pressure, or seizure
What to do
- Call emergency services for fever, rigidity, severe agitation, or seizure
- Keep the person cool and in a calm, quiet space while waiting
- Do not combine DXM with any serotonergic medication (SSRIs, SNRIs, MAOIs, tramadol, St John's wort)
- If you are on an SSRI, recreational DXM is off the table — pick a different night for everything
