Kratom / 7-OH + Opioids: the short answerKratom's key alkaloids — especially 7-hydroxymitragynine, now sold in concentrated '7-OH' products — act on the same opioid receptors as heroin and oxycodone, so combining them stacks respiratory depression. Treat unresponsiveness as an opioid overdose: naloxone and an ambulance.

Why it’s dangerous

Mitragynine and 7-hydroxymitragynine are partial-to-full mu-opioid receptor agonists — pharmacologically, kratom is an opioid. Concentrated 7-OH products are far more potent than leaf and carry real overdose risk on their own. Added to prescription opioids, heroin, or fentanyl, the breathing suppression adds up, and because kratom products vary wildly in strength, the effective combined dose is impossible to predict.

What happens

  • Much deeper sedation and 'nodding' than expected
  • Slow, shallow breathing
  • Nausea and vomiting with aspiration risk
  • Overdose deaths in which kratom plus another opioid are found together

Warning signs

  • Cannot be woken; pinpoint pupils
  • Breathing under ~8 breaths a minute or with long pauses
  • Blue lips or fingertips

What to do

  • Call emergency services and give naloxone — it works on kratom's opioid effects too; repeat as needed
  • Recovery position; rescue breathing if trained
  • Do not use kratom or 7-OH to 'stretch' or boost other opioids
  • Be aware concentrated 7-OH products are dependency-forming and far riskier than traditional leaf
