MAOIs + MDMA
MAOIs + MDMA: the short answerTaking MDMA while on an MAOI (or within two weeks of one) can cause fatal serotonin syndrome and extreme overheating. This is one of the most dangerous drug combinations known — avoid it completely.
Why it’s dangerous
MDMA works by forcing massive serotonin release; MAOIs disable the enzyme that would normally clear that serotonin away. The result is a runaway serotonin surge far beyond a normal MDMA experience, with body temperature, heart rate, and blood pressure rising to life-threatening levels. The same logic applies to pharmaceutical MAOIs and to harmala alkaloids (as in ayahuasca).
What happens
- Serotonin syndrome: extreme agitation, confusion, rigid jerking muscles
- Dangerously high body temperature (above 40°C/104°F)
- Very high blood pressure and racing or irregular heartbeat
- Seizures, organ failure, and death
Warning signs
- Feeling far 'higher' than expected, with panic and confusion
- Profuse sweating, hot skin, tremor, or jaw clenching beyond usual
- Fainting, seizure, or temperature that keeps climbing
What to do
- Call emergency services immediately — cooling and hospital care save lives here
- Move the person somewhere cool, loosen clothing, apply cool water while waiting
- Do not give more of anything, including 'a benzo to calm down'
- If you take an MAOI, MDMA and similar drugs (MDA, 5-MAPB-type compounds) are strictly off-limits — including for ~2 weeks after stopping the MAOI
