Can you overdose on MDMA? The signs, the 'lethal dose' myth, and what to do

Yes — and it usually doesn't look like an opioid overdose

MDMA deaths are rare relative to use, but they follow a predictable pattern every summer: overheating, water imbalance, and stacked doses. Unlike fentanyl, MDMA rarely just 'stops your breathing' — it cooks you from the inside and floods your brain with serotonin.

The water trap

MDMA makes you hot, thirsty, and water-retaining (it triggers ADH). Drink too much water — several liters — and your blood sodium crashes. The result, hyponatremia, kills festival-goers who did everything 'right' except the water math. Sip electrolyte drinks, don't chug water. Roughly 500 mL per hour max, more if dancing hard in heat — and eat something salty.

The 'lethal dose' myth

There is no reliable single lethal dose number — it varies wildly by person, temperature, activity, and what's actually in the pill. What the data shows:

  • - Deaths cluster at high doses (200 mg+ in a session), hot environments, and redosing
  • - The risk compounds: dose 1 + heat + dose 2 at 2 AM is the classic fatal pattern
  • - Pills sold as MDMA sometimes contain PMA/PMMA — slower to hit, deadly when redosed. A pill that takes 2+ hours to work is a red flag, not a weak pill

What to do in an MDMA emergency

  1. 1. Call emergency services — say 'possible drug overdose, high fever, confused.' Good Samaritan protections apply in most places
  2. 2. Cool them actively: shade, strip layers, fan, cool damp cloths on neck/groin/armpits. Heat is what kills
  3. 3. Sips of electrolyte drink only — not gulps of water
  4. 4. If seizing: protect the head, nothing in the mouth, time it
  5. 5. Tell responders it was MDMA — they can treat serotonin syndrome and hyperthermia, but only if they know

The prevention rules that actually work

  • - Test your drugs (reagent kits distinguish MDMA from PMA for ~$20)
  • - One dose, then hands off: never redose on a comedown
  • - Rest and cool down between dances; the danger is heat + duration, not just the pill
  • - Tell your friends what you took and when — the person who can call for help at 3 AM is your real safety equipment
